Request Forms
Schools are responsible to make all reservations and arrangements prior to submitting requests for Arts in Education (AIE) and Exploratory Enrichment (EE). Erie 1 BOCES oversees and prepares the contracts and payments only. This process can take 6-8 weeks in some cases, so you must fill out this form well ahead of the program date.
Extracurricular Activity requests only consist of eSports and CYPRAS (Masterminds & Chess) at this time.
Request Form Requirements:
- All requests must have a clear connection to the arts standards (AIE) or content standards (EE)
- For student experiences, all requests must show evidence of direct and clear impacts on student learning (AIE & EE)
- For staff development, all requests must have a direct and clear impact on teacher learning (AIE). Exploratory Enrichment (EE) does NOT support staff development.
- Arts in Education (AIE) requests may support both school day and after school activities. Exploratory Enrichment (EE) may only support school day.
- Program requests cannot include food, travel, or lodging (AIE & EE)
- Consultants are allowed to have contact with students only when a district employee is present. Your submission of this form indicates that your school personnel understand this and that consultants in your school will not be asked to violate this rule.
- If you are submitting an Arts in Education (AIE) request with MTI Enterprises for school musical licensing, please read and follow the directions linked below.
Cancellations: It is the responsibility of the school to contact Erie 1 BOCES Arts in Ed ASAP if the program does not take place. Erie 1 BOCES will pay an artist or vendor unless we hear otherwise from the school that the program/event was cancelled or rescheduled.
The deadline for all requests for the 2025-2026 school year is May 15, 2026.
